 AYA Medical-Legal Partnership

A Legal Support System for Young Adults with Cancer

The AYA@USC program is the newest oncology program at the USC Keck School of Medicine that focuses on treatment and care for adolescents and young adults with cancer. The program aims to provide high quality health care while addressing patients’ psychosocial needs and is led by USC physicians with both pediatric and adult oncological expertise. While many patients have benefited from the program, some continued to struggle due to a lack of socio-economic resources. In order to meet the needs of these young adults, AYA@USC partnered with Neighborhood Legal Services of Los Angeles County (NLSLA) in order to provide them with a strong support system. As a result, a Medical Legal Community Partnership (MLCP) was created.

Initially started by Barbara and Stuart Siegel, the idea of a medical legal partnership was born in Boston and later came to Los Angeles to further emphasize the importance of community. This unique partnership seeks to view legal issues holistically and connects professional disciplines, health providers, and lawyers in order to improve patients’ health statuses. Through the MLCP, the AYA program was able to focus more on the social determinants of health and find ways to reduce barriers patients living in poverty face.

Kyle, a 20-year-old who could not afford the $500 copayment associated with his cancer treatment is an example of an AYA patient who struggled to find affordable healthcare coverage. NLSLA evaluated several options for him before finding that he was eligible for a Medi-Cal program called Hospital Presumptive Eligibility (HPE). HPE enables patients who may be Medi-Cal qualified (but have yet to be confirmed) receive Medi-Cal immediately before they submit and get approval from a normal Medi-Cal application that would otherwise take 45 days. Kyle was also told that he may qualify for benefits that would cover the co-insurance costs of the first two months of his treatment. By assisting with his medical coverage, NLSLA was able to have a direct impact on an AYA patient that resulted in avoidance of treatment delays and anxiety regarding affordable medical care.

NLSLA continues to work very closely with the AYA team, training members on a wide array of issues such as how applications for public benefits work, Medi-Cal, available options for subsidized housing, immigration issues, job transitioning options, and healthcare benefits. As of 2019, over 109 patients have been assisted by NLSLA through the MLCP. Together with the AYA@USC, NLSLA has established procedures for the MLCP which remain in use today. As a result, the medical team has grown much more knowledgeable about the social determinants of health and has more effectively helped its patients. In addition, NLSLA has not only improved healthcare access for individual patients but has also had a larger impact for thousands of future patients.

One area that has gotten more attention in the cancer community is fertility preservation services. Previously, health plans did not cover fertility preservation services and only offered limited reimbursements. In order to advocate for increased access to fertility treatments, NLSLA represented AYA@USC and helped patients receive thousands of dollars in order to receive these services. Most recently, an attorney working within NLSLA was able to get California legislation to mandate fertility preservation coverage by all insurance providers.

MLCP is a significant achievement that continues to help low-income families and children dealing with cancer in Los Angeles County. From health to housing, access to justice and economic security, NLSLA has been able to support the AYA community through its MLCP in order to make their lives better.
